 JOINING THE PRACTICE

We normally accept patients from Lichfield and Burntwood, who live within our practice area.

Before you register please

1) Check whether you live within our practice area (our receptionists will be able to tell you)

2) Read our practice charter, and read through the information contained on this website or within our practice leaflet to ensure that you feel that we are a suitable practice for you

3) Check that there are places available

 

On registration you will be invited for a health check, usually with our practice nurse. It is important to attend as it helps to plan the care you might need. Please bring a urine sample with you when you attend.
